Records Retention

Special Education records (including placement records, referrals, evaluations, testing data, and other related records) must be kept by the school district for four years after a student's final year of enrollment in Special Services programs.

Records Requests

If you wish to receive a copy of your son/daughter's file, please call 928-505-6955 between 8 a.m. and 3 p.m.

All requests for special education records require a parent or guardian signature. (Note: Once a student turns 18, the records will only be given to them, and not a parent. There may be exceptions. If you have any questions, give us a call.)

Please allow up to 5 business days for requests to be processed. You will receive a phone call when records are ready for pick up. Records will be available for pick up Monday–Friday, 8:00–3:00 the school district office (2200 Havasupai Blvd., Building C)

A records request form may be obtained at the district office, Building A, or you may download the form. You will need to bring the completed request form and a picture ID with you when you pick up the records. Thank you!

Record Retention

During May/June of 2020, we will be shredding Special Education files for students who graduated, moved out of the District, or terminated from Special Services in 2015-16.
