Full primary care, limited urgent care, and wellness services are available. Walk-ins are welcome. Appointments can be made by phone (928-854-6120) or online.

Normal hours:

  • Sunday Closed
  • Monday 7–5
  • Tuesday 7–5
  • Wednesday 12–6 (8-2 during holiday breaks and summer break)
  • Thursday 7–5
  • Friday 7–5
  • Saturday 7–12


NOTICE: As of July 01, 2018, non-preventive services and prescriptions provided at the Health & Wellness Center will be subject to co-payments* as follows:

Non-Preventive Provider Visits (including sports physicals)
$20 co-pay

Non-Preventive Medications
$10 co-pay

Co-payments must be made with a debit or credit card ONLY at the time of service. 
No cash or checks will be accepted.

*Members can obtain a receipt from the Center and submit that to Gilsbar for application toward their deductible.
